Care Coordinator Community care direct is a Nurse led organisation who are immensely proud of the support which we provide families and individuals to live independently in their own homes. Community Care Direct is a well established and highly respected local independent care provider, working in unison with service users to provide high quality care in their own homes. Through regular close contact, we aim to provide person centred care which adapts to the evolving care needs of our service users. Working under the direction of the care manager you will be managing the day to day running of your team, ensuring your service users receive safe, compliant and legal care. The position is office and community care based. Must live locally as there is an on call rota. We are currently looking to expand our team and have two vacancies for Care Co- Ordinator’s. The following is not an exhaustive list. Duties and responsibilities to include:

To undertake risk assessments of the service user, including the care environment and all equipment.
To set new care packages in line with company and regulatory requirements to ensure that they are ready for service delivery.
To observe the confidentiality of service users and staff at all times.
Assess, reassess and amend service user risk assessments and care assessment, acting upon changes which have been reported to the office.
To undertake alternative weekend on call duties on a rota basis.
To attend regular meetings within the office and care teams.
To routinely attend service user reviews as requested by the care manager. Person Specification:
An understanding of CQC regulations in relation to domiciliary care.
Effective communication skills.
Knowledge of office operating systems.
Ability to flexible in working approach.
An in depth knowledge of domiciliary care related issues.
A positive professional working attitude.
A commitment to continual professional development.
Applicants must hold a relevant Level 3 qualification or be willing to undertake within 6 months of appointment.
Must be a car driver
