The service you will work in….

The Grove Outreach is a vibrant and busy service which enables Royal Borough of Kensington & Chelsea residents to avoid, and in particular to reduce the need for, prolonged hospital admissions and long-term dependency on mental health services. The Grove delivers community based, person-centred and holistic support to service users aged over 18, who have severe or enduring complex needs.

We provide a weekly range of creative activities including art, pottery and social discussion groups. We are a rewarding service in which we work closely in coproduction with our service users in order to increase their independence and combat isolation.

A day in this role looks like….
\n
As a Senior Group Facilitatorat Hestia, you will play a crucial role in our Integrated Mental Health Service. Your contributions will directly influence the lives of our service users, guiding them towards a path of empowerment and independence.
\n
A day will never look the same and some of the things you can expect to do/be responsible for are:
\n
- \n
- To be responsible for the support and well-being of a group of key service users allocated from across the team using holistic, person-centred support planning with SMART targets and the recovery star using the recovery approach to enable them to increase independence, maintain their accommodation and enjoy a good quality of life.\n
- To visit service users for support sessions within their homes or appropriate community locations.\n
- To devise effective interventions for service users who are ‘hard to engage'.\n
- To enable the service users to access medical services and the local community mainstream services.\n
- To ensure that allocated service users have a needs & risk assessment and SMART support plan; which are reviewed at agreed frequencies.\n
- To minimise unplanned and emergency hospital admissions for mental health relapse. To formulate, implement and review support plans with service users and associated professional participation.\n
- To liaise effectively with other agencies involved in the support of service users where appropriate.\n
- To encourage users to have on-going engagement with existing networks including faithbased groups and informal sources of support.\n
- To provide information and advice on benefits, housing and welfare issues to service users.\n
- To facilitate access to self-management programmes.\n
- To build strong links with community organisations and work with them to support wider engagement with service users.\n
- To work with universal services to ensure users have access to appropriate community-based facilities, promoting community cohesion.\n

- Experience of working with people requiring support related to mental ill health and complex needs.\n
- Experience of delivering support to people within a community/outreach setting.\n
- Ability to support service users to achieve their goals & aspirations.\n
- Excellent knowledge of, issues related to signs, symptoms and treatments of mental illness; ability to assess service users declining mental health and take appropriate action.\n
- Knowledge of health and safety relevant to the role including working within a community setting.\n
- Knowledge of key working, person centered support planning systems and the recovery approach.\n
- An understanding of equality and diversity and how it applies to the job role and the importance of promoting service users' rights, privacy dignity and choice.\n
- Knowledge of welfare benefits entitlements appropriate to the user group.\n
- Knowledge and understanding of safeguarding issues and ability to address them appropriately.\n
