Responsibilities:
- Support the local authorities Cared for and Care Experienced Young People as they move towards independence
- Work with the children and young people on their pathway plan that looks at their health, education, employment, training and relationships
To be considered for a role within the Pathways Team, ideally you will have:
- BSc in Social Work or equivalent
- Social Work England registration
- At least 1 year of experience within a similar children's social work setting
