Personal Advisor – Leaving Care Team
Reigate
Pay- £19.49 – £25 per hour
6 months initially
Monday to Friday, 37 hrs/week (may be required to carry out duty on a rota basis alongside existing workload)
Manage a caseload of young people leaving care aged 18 – 25 years
Support the young people with the transition into adulthood and independent living, ensuring they are equipped to make good decisions and have the practical skills necessary to live independently
Conduct home visits and keep in constant contact with the young people
Assess risk and complete risk assessments
Participate in the preparation of pathway plans
Support the young person in achieving the outcomes outlined within their pathway plan through provision of relevant information, advice, support and signposting to relevant services and partner agencies
Undertake Subject Access Requests
Advocate on behalf of the young people where necessary
Maintain up to date knowledge in respect of education, employment, apprenticeships, housing, welfare entitlements and asylum issues
Ensure commitment to ongoing training and development Requirements
Minimum of a relevant NVQ Level 3 /4 qualification
Practical knowledge of relevant legislation, processes and procedures
Ability to assess, plan and review cases; undertake challenging casework, where appropriate shadowing more experienced social workers/practitioners
An awareness of current issues relating to socially excluded young people
