On Call Care Support Co-ordinator – Home Based (Bristol) - £Competitive hourly rate.
2 evenings a week (ideally Tuesday and Wednesday). Every second weekend (weekend hours are Friday 5pm until 8am Monday)

The primary objective of this role is to provide a reliable, flexible, efficient, and professional service to our Service Users, their relatives/advocates, our team of Homecare Support Workers, and healthcare professionals.

What will the role involve as a On Call Care Support Co-ordinator?
Service User Condition Reporting: Report any changes in the Service User’s condition or circumstances.
Communication: Liaise regularly with the Registered Manager, Care Managers, Service Users, their advocates/families, and Homecare Support Workers.
Rota Management: Inform the Rota Manager/Rota Administrator of any situation affecting the rota, reallocate work runs during carer sickness, and cover essential calls when necessary.
Emergency Response: Respond to emergency situations calmly and professionally, ensuring Head Office is notified promptly.
Documentation and Confidentiality: Maintain accurate documentation, update Client Service Orders, ensure Homecare Support Workers complete required records, and uphold confidentiality for all parties.
