Job Summary: The Support Coordinator will play a pivotal role in the day-to-day operations of our semi-residential living provisions. This position involves coordinating and delivering comprehensive support services to young people, ensuring their individual needs are met, and fostering a safe, inclusive, and positive living environment. The Support Coordinator will work closely with residents, their families, and a multidisciplinary team to create and implement personalised care plans.
Key Responsibilities:

Resident Support: Provide direct support to the young people, including emotional, social, and practical assistance, to help them achieve their personal goals and enhance their quality of life.

Care Planning: Develop, implement, and review individual care plans in collaboration with the young people, their families, and other professionals. Ensure that care plans are tailored to meet each young person’s unique needs and aspirations.

Coordination of Services: Liaise with external agencies, healthcare providers, educational institutions, and other relevant organisations to coordinate comprehensive support services for the young people.

Health and Safety: Ensure the safety and wellbeing of the young people by adhering to health and safety regulations and facility policies. Conduct regular risk assessments and implement safeguarding procedures.

Record Keeping: Maintain accurate and up-to-date records of young people’s progress, care plans, and any incidents or concerns. Ensure confidentiality and compliance with data protection regulations.

Team Collaboration: Work collaboratively with other staff members, attending team meetings and contributing to the development and improvement of services.

Training and Development: Participate in ongoing training and professional development to stay updated with best practices in semi-residential living facilities.

Crisis Management: Respond effectively to emergencies and challenging situations, providing support and guidance to the young people and staff as needed.
Qualifications and Experience:

Education: A relevant qualification in social work, healthcare, or a related field (e.g., NVQ Level 3 in Health and Social Care).

Experience: Previous experience in a support or coordination role within a supported living or similar care setting, particularly working with young people.

Excellent organisational and time management skills.

Empathy, patience, and a genuine passion for supporting young people.

Knowledge: Understanding of relevant legislation, policies, and best practices in supported living care.

Other: A valid UK driving license and the ability to travel is desirable. Right to work in the UK
