Royal Voluntary Service has an opportunity available for a SERVICE COORDINATOR to join our team. You will join us on a part-time, permanent basis, and in return, you will receive a competitive salary of £12,974.85 per annum (£22,706 FTE), combined with homebased working with travelling within Dundee City.
About the SERVICE COORDINATOR role:
The role has responsibility for the Coordination of a Supporting Your Recovery Service within a Home from Hospital support service. Under the Supporting Your Recovery operational model the support offered through staff and Volunteers commences within the community as the person is discharged from Hospital. With a primary focus on Nutrition, Hydration, and both cognitive and physical exercises, the patient is supported to remain in good health and out of hospital.
Once home, the Home from Hospital element of support continues for up to 6 weeks, helping people reconnect with their community or activities of interest. The service aims to reduce recovery time following a hospital stay, reduce readmissions and reduce the overall impact of isolation and loneliness in older people.
The role includes supporting the Service Manager in recruitment, training and support of Volunteers, receiving and action referrals from local stakeholders and ensuring the service is delivered in line with any contractual agreement and with Royal Voluntary Service Vision and Core Values in order to achieve key performance targets
Hours: 20 Hours to be worked across 3 or 4 days, to be agreed but likely to include a Friday.

What you'll bring as our SERVICE COORDINATOR:
A working knowledge and understanding of the NHS, Commissioning groups and local authority Commissioning including Health and Social Care bodies.
Demonstrable experience of working in a person-centred manner.
Awareness of the principles of safeguarding (Adults and Children)
Excellent communications ability at all levels.
Ability to act on own initiative within agreed parameters.
Ability to proficiently use standard IT systems.
Additonal Information:
The ability to regularly travel across the Dundee City area to conduct Initial Assessments and reviews.
Full UK driving license (essential)
