Job Title: Extra Care Coordinator

About Us: Anational not-for-profit organisation providing person-centred services. We support people with a wide range of needs, including dementia, mild learning disabilities, mental health needs, and older people.
The Role: We are seeking a dedicated Extra Care Coordinator to manage and ensure the quality of services across Burnside Court and Heysham Gardens. This role involves collaborating with Social Services and other stakeholders to ensure that CQC care standards are met and the highest level of customer service is provided.
Key Responsibilities:

Operate and manage services at both sites, maintaining high-quality care standards.

Collaborate with housing providers and families to promote a positive atmosphere.

Lead the delivery of flexible, person-centred services.

Ensure respectful personal care and support for clients.

Recruit, supervise, and support staff to maintain service standards.

Promote social activities and community connections for service users.
-
Maintain administrative and financial control systems.

Ensure effective joint working with partner agencies.
Service Management:

Promote excellent working relationships with clients, families, and professionals.

Deliver flexible, person-centred services that respect clients' privacy, dignity, and independence.

Support clients in activities that sustain a positive quality of life.

Ensure that staff resources meet the assessed needs of clients.

Develop and maintain care plans that are responsive to changing needs.

Ensure that clients receive high-quality care from internal and external services.

Support clients in managing their health and wellbeing.
Requirements:
-
Significant experience in Extra Care or Home Care services, Residential day or community services, Prevention and re-enablement services.

Diploma in Health and Social Care, Level 3 (Essential).

A degree level or post-graduate qualification (Desirable).

Ability to assess needs and devise and implement outcome-focused care/support plans.

Strong communication skills and the ability to produce high-quality written reports.

Terms and Conditions:

Full-time (37.5 hours per week) or part-time (30 hours per week) including weekends and public holidays.

25 days annual leave plus eight statutory days pro-rata.

Birthday Holiday Bonus after two years of continuous service.

Probationary period of four months with a £100 bonus upon successful completion.
