Job Description:
As an Adults Mental Health Social Worker, you will play a crucial role in assessing, planning, and delivering support to individuals experiencing mental health challenges. You will work within a multidisciplinary team to provide holistic care, promote recovery, and enhance the quality of life for service users.
Key Responsibilities:
Assessment and Care Planning:
Conduct thorough assessments of individuals' mental health needs, including risk assessments and evaluations of social, emotional, and environmental factors.
Develop and implement individualised care plans in collaboration with service users, their families, and other professionals, focusing on recovery-oriented goals.
Direct Support and Intervention:
Provide therapeutic support, counselling, and crisis intervention to individuals experiencing mental health issues.
Advocate for service users' rights and needs, ensuring they have access to appropriate services and resources.
Multi-Agency Collaboration:
Work closely with healthcare providers, community services, housing agencies, and other relevant organisations to coordinate and deliver integrated care.
Participate in multidisciplinary team meetings, case conferences, and care reviews to ensure comprehensive and cohesive support.
Safeguarding and Risk Management:
Identify and respond to safeguarding concerns, following established protocols to protect vulnerable adults from harm.
